POST Personal History Statements

 
 

 

Two new POST Personal History Statements are available:

These forms were originally approved by the POST Commission in April 2005. However, they have been revised and reissued in February 2006 to better ensure full compliance with the pre-offer inquiry prohibitions of the Americans with Disabilities Act (ADA) and the California Fair Employment & Housing Act (FEHA). The revised versions no longer ask respondents to indicate frequency, extent of use, or time periods associated with past recreational drug use. Investigations into the extent and nature of past illegal drug use must be deferred until the post-offer stage.

Although the dispatcher and peace officer PHS contain many of the same questions, there are several key differences as a result of both legal statutes and the different demands and requirements associated with these two professions.

These forms can be downloaded and completed by hand, or filled in electronically (except for initials and signatures).

The forms available here are “protected,” that is, they cannot be altered. However, agencies who wish to modify these forms to fit the specific needs of their organization can request unprotected forms from Mechelle Schultz at 916.227.4820 or by email at Mechelle.schultz@post.ca.gov.
